Director of Nursing (Skilled Nursing Facility)

Middle River Health and Rehabilitation Center is an 86-bed skilled nursing and rehab facility with a strong quality reputation and a team that sticks around. A lot of our staff have been here for years, and we're proud of the culture behind that.

We're looking for an experienced Director of Nursing to lead our nursing department. Someone who can build up a strong team, keep us survey-ready, and hold the standard of care our community counts on.

What you bring:

• Active RN license in Wisconsin

• Nursing leadership experience in skilled nursing or long-term care

• Working knowledge of state and CMS regulations, QAPI, and the survey process
