MDS Coordinator
Job Summary:
We are seeking a dedicated and compassionate Registered Nurse MDS Coordinator to join our team at Fulton Manor Care Center, a leading provider of Short Term Therapy and Long Term Care in Fulton, MO. As an integral member of our care team, the MDS Coordinator will play a vital role in coordinating the care of our residents, ensuring that their physical, emotional, and social needs are met.
About the Role:
Coordinate the care planning process for residents, collaborating with healthcare professionals, families, and other stakeholders to develop and implement individualized care plans
Assist with assessing and documenting resident needs, preferences, and goals
Facilitate effective communication among healthcare professionals, families, and other care providers to ensure seamless care coordination
Identify and address any gaps or inconsistencies in care, working to resolve issues promptly and efficiently
Maintain accurate and up-to-date records of resident care, including MDS assessments, care plans, and progress notes
Responsibilities:
Collaborate with the interdisciplinary team to develop and implement resident care plans
Conduct MDS assessments and develop care plans in accordance with current regulations and standards
Assist with implementing and evaluating resident care programs and services
Participate in quality improvement initiatives to improve care outcomes and resident satisfaction
Maintain confidentiality and adhere to HIPAA guidelines in handling resident information
Perform other duties as assigned by the Director of Nursing or other administrative staff
Requirements:
RN licensure with current registration in the State of Missouri
MDS Coordinator experience or related experience in healthcare setting
Strong communication and interpersonal skills, with the ability to work effectively with diverse populations and caregivers
Proficiency with electronic medical records (EMRs) and other healthcare software
Ability to work in a fast-paced environment, prioritizing multiple tasks and competing demands
Strong organizational and time management skills, with attention to detail and accuracy
