REGISTERED NURSE

Job Responsibilities Include but are not limited to:

· Performing admission assessments and reassessments for all residents in the home

· Developing, monitoring, and reevaluating plans of care

· Day to Day oversight of personal care aides

· Interacting with primary care, specialty and hospital providers to ensure resident needs are being met

· Following all regulatory guidelines 

· Triaging and assessing residents to determine healthcare needs

· Participating in continuous quality improvement program, quality assurance activities and regular chart audits

Job Qualifications:

· Active NYS Registered Nurse License

· 1-3 years experience in an Assisted Living or Long Term care setting, high preferred but not required

· Experience with developing plans of care preferred

· Experience with various assessments such as UAS, OASIS, PRI & Screen preferred

 $80,000 - $100,000 Annually
