Compensation: $25.00 – $30.00 per hour (DOE)
Position Overview
Grace Care Homes, Inc. is seeking a highly organized Program Coordinator to assist with the daily operations of our Residential Treatment Home and Adult Foster Homes, which serve individuals with severe and persistent mental health. This individual will work closely with the Program Administrator, providing assistance with care coordination, staff development and oversight, and ensuring compliance with Oregon Administrative Rules (OARs).
The Program Coordinator will help oversee resident care documentation, supervise direct care staff, monitor medications through the electronic MAR (Medication Administration Record) system, manage scheduling, and maintain records. In addition, this individual must be willing and able to cover direct care staff shifts when necessary to maintain safe and consistent resident support.
This role requires advanced proficiency in Microsoft 365 including Teams, Excel, Word, and PowerPoint for documentation, scheduling, communication, reporting, and training. In addition, experience with healthcare documentation systems is required.

Key Responsibilities
- Assist the Program Administrator in executing duties mandated by Oregon’s Administrative Rules for RTH and AFH
- Ensure compliance with Oregon Administrative Rules (OARs), licensing requirements, and Grace Care Homes policies and procedures.
- Support staff scheduling, training, evaluations, and supervision.
- Ensure resident rights, dignity, and independence are upheld.
- Participate in quality assurance processes, audits, inspections and incident investigations.
- Prepare professional reports, spreadsheets, and presentations using Microsoft Excel, Word, and PowerPoint.
- Utilize Microsoft Teams for internal communication and collaboration.
- Maintain accurate and up to date resident records, progress notes, and other required documentation.
- Assist with administrative and operational tasks as needed.
- Cover direct care shifts when necessary to ensure staff consistency and resident safety.
- Experience working in a Residential Treatment Home (RTH), Adult Foster Home (AFH), or similar mental health care (preferred).
- Knowledge of Oregon Administrative Rules for RTH/AFH programs.
- Strong leadership, organizational, and communication skills.
- Advanced proficiency in Microsoft 365 (Teams, Excel, Word, PowerPoint) required.
- Working knowledge of Point Click Care (PCC) or other EHR/medication documentation systems.
- Must meet the qualifications of a Direct Care Staff, including the ability to complete all required training and certifications.
- CPR/First Aid, or ability to obtain.
- QMHA certification is a plus but not required.
Schedule & Compensation
- Full-time position with flexible scheduling to meet operational needs.
- Hourly rate: $25.00 – $30.00 DOE.
- Must be available to work evenings, weekends, or holidays if required for shift coverage.
