DESCRIPTION OF WORK

This role supports older adults and eligible individuals by completing assessments, arranging services, and providing clear information about available programs. The work helps consumers access resources, understand options, and maintain their independence in a safe and informed way. As an Aging Care Manager 2, you will perform the following duties:

  • Intake Support: Collect consumer information, document details, and explain available programs and eligibility
  • Service Coordination: Arrange services for consumers based on simple or complex needs in cooperation with community resources
  • Assessment Duties: Conduct evaluations in homes, facilities, or other settings to determine service needs and access to resources
  • Case Management: Maintain caseloads, complete reassessments, and gather financial and personal information when required
  • Communication Tasks: Prepare letters, reports, and records while responding to inquiries from families and professionals
  • Record Maintenance: Track daily activities, update databases, and maintain confidential information with accuracy


Work Schedule and Additional Information:

  • Full-time employment
  • Work hours are 8:00 AM to 4:30 PM, Monday - Friday, with 60-minute lunch.
  • On-Call Hours When Required. Employee may have overnight travel or out of county travel for meetings or training.
  • Telework: You will not have the option to telework in this position.
  • Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • Six months as an Aging Care Manager 1 or a County Caseworker 1; or
  • Successful completion of the County Social Casework Intern program; or
  • Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
  • An equivalent combination of experience and training.

  • You must be able to perform essential job functions.

  • You must pass a background investigation.
  • A conditional offer of employment will require a drug screening.
  • This position falls under the provisions of the Child Protective Services Law.
