Job Description
The Case Manager Special Child Health Services plays a vital role in supporting children with special health care needs and their families by assessing needs, developing individualized service plans, coordinating care, and facilitating communication among providers and agencies. This position involves managing a caseload within a collaborative team structure and reporting to both a Program Manager and the Health Services Director. The work mode is hybrid, combining field visits and office work, with frequent travel within the county and occasional travel to regional offices.
Responsibilities
- Assess the needs of the child and family to determine appropriate services
- Develop and document an Individual Service Plan (ISP) addressing developmental, educational, medical, rehabilitative, economic, and social/emotional needs
- Assist families in achieving ISP goals through resource procurement and referrals
- Provide crisis support and respond to complaints during business hours
- Facilitate communication among multiple providers, including primary care physicians and medical homes
- Monitor and review progress toward ISP goals and adjust plans as needed
- Establish relationships with local and regional agencies serving children with special health care needs
- Maintain thorough documentation of family communication attempts and case activities
- Avoid and manage conflicts of interest with transparency and professional discretion
- Assist families in applying for relevant financial assistance programs, including FCCS Fee-For-Service and CICRF
- Transfer case records appropriately when children relocate to other counties within New Jersey
Required Qualifications
- Bachelor's or Master’s degree in nursing (BSN/MSN) or social work (BSW/MSW)
- At least one year of pediatric case management experience for children with special health care needs (CSHCN)
- Equivalent degree in a related field considered on a case-by-case basis with licensure requirements determined accordingly
Preferred Qualifications
- Valid New Jersey nursing license (RN) or social work certification/license (CSW, LSW, LCSW)
- Experience in pediatric case management and individual service planning
- Skills in resource coordination, crisis intervention, and effective communication
- Proficient in documentation, reporting, and family support
- Ability to manage conflicts of interest transparently
- Knowledge of program eligibility assistance and interagency collaboration
- Experience working with adolescent case management
