Registered Nurse Case Manager

We are seeking a dedicated and Registered Nurse Case Manager to coordinate and oversee patient care across diverse healthcare settings. As a Registered Nurse Case Manager, you will collaborate closely with healthcare professionals to ensure optimal patient outcomes while adhering to health regulation policies and maintaining compliance with HIPAA standards.

Responsibilities

• Completing the initial nursing assessment and developing an individualized plan of care.
• Perform regular skilled nursing visits to assess the patient's condition and progress.
• Supervising and directing License Practical Nurses, Registered Nurses and Certified Home Health Aides as required by state regulations and agency policy.
• Manage utilization review processes by evaluating medical records, and treatment plans to ensure appropriate resource utilization.
• Collaborate effectively with multidisciplinary teams including physicians, therapists, social workers, and other healthcare professionals to facilitate seamless patient care transitions.
• Maintain accurate documentation while ensuring compliance with HIPAA regulations and medical terminology standards.
• Participate in quality improvement initiatives.
