Licensed Social Worker (LSW)

Are you a Licensed Social Worker who wants to do more than complete paperwork? We are seeking a compassionate, motivated LSW to join our skilled nursing and long-term care team in the Chillicothe area.

This is an opportunity to build meaningful relationships with residents and families, advocate for their needs, and play an important role in their care from admission through discharge. You’ll work alongside nursing, therapy, activities, dietary, and facility leadership as part of a collaborative interdisciplinary team focused on providing quality, resident-centered care.

What You’ll Do:

• Serve as an advocate and resource for residents and their families

• Build relationships with residents and support their emotional, psychosocial, and social needs

• Coordinate admissions, discharge planning, and safe transitions of care

• Complete psychosocial assessments, care plans, MDS-related documentation, and progress notes

• Participate in care conferences and interdisciplinary team meetings

• Assist residents and families with community resources, benefits, services, and placement needs

• Support residents and families through changes in condition, end-of-life care, and other challenging transitions

• Address resident concerns and help ensure resident rights are protected

• Collaborate with nursing, therapy, hospice, behavioral health, and other community partners

• Maintain accurate documentation and compliance with state and federal long-term care requirements

What We’re Looking For:

• Current Ohio Social Work License (LSW) required

• Long-term care, skilled nursing, healthcare, or geriatric experience preferred

• Knowledge of MDS, PASRR, care planning, and discharge planning is a plus

• PointClickCare (PCC) experience preferred

• Strong communication, organization, and problem-solving skills

• Someone who is comfortable working independently while also being an active part of the facility team

• Most importantly, someone who genuinely enjoys working with seniors and believes in being a strong advocate for residents
