Job Summary

Stellar Health Group is seeking a dedicated and compassionate Occupational Therapist (OT) to join our energetic and collaborative therapy team.

Responsibilities
  • Provide occupational therapy services that promote excellence in rehabilitation care, interdisciplinary cooperation, and quality patient outcomes.

  • Promote the facility's geriatric rehabilitation program to medical professionals and the community.

  • Evaluate patients in a timely manner according to state, payer, and facility requirements.

  • Screen patients upon admission and whenever requested by the Director.

  • Develop individualized treatment plans and appropriate goals based on patient evaluations.

  • Provide treatment according to the established plan of care.

  • Communicate and collaborate with other disciplines and facility staff as indicated by the treatment plan.

  • Participate in family conferences when requested.

  • Educate and counsel patients, families, and staff regarding the rehabilitation process as appropriate.

  • Complete routine documentation in accordance with Medicare, facility, and other payer guidelines.

  • Record and submit all required documentation and treatment charges accurately and in a timely manner.

  • Participate in discharge planning and interdisciplinary discussions as needed.

Qualifications
  • Current Massachusetts state license as an Occupational Therapist.

  • Ability to work collaboratively as part of a dynamic and cooperative interdisciplinary team.

  • Flexibility to distribute workload according to departmental needs and individual responsibilities.

  • Strong evaluation and assessment skills.

  • Ability to develop individualized treatment plans and goals.

  • Ability to participate in consultations, teaching, and meetings with residents, staff, families, and other healthcare professionals.

  • Strong communication and interpersonal skills.

  • Ability to educate and counsel patients and staff regarding occupational therapy and rehabilitation needs.
