Schedule: 5 × 8-Hour Shifts Per Week
Shift Time: 2:00 PM – 10:00 PM
Hours: 40 Hours Per Week

Position SummaryWe are seeking an experienced Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) to provide direct nursing care to residents in a long-term care setting. The nurse will assess resident needs, administer medications and treatments, monitor changes in condition, maintain accurate clinical documentation, and collaborate with the interdisciplinary healthcare team.
The ideal candidate will have at least 1 year of nursing experience, strong long-term care or skilled nursing experience, and the ability to effectively manage assigned resident workloads.
Responsibilities- Provide safe, compassionate, and comprehensive nursing care to long-term care residents.
- Perform nursing assessments and monitor residents for changes in condition.
- Administer medications and treatments accurately according to provider orders and facility policies.
- Care for residents with dementia, arthritis, congestive heart failure (CHF), cerebrovascular accident (CVA), and other chronic conditions.
- Monitor vital signs, symptoms, and treatment responses and report significant changes promptly.
- Assist with developing and implementing individualized resident care plans.
- Accurately document assessments, medications, treatments, interventions, and resident responses in the Epic electronic medical record.
- Communicate effectively with physicians, RNs, nursing assistants, residents, families, and other healthcare professionals.
- Provide education to residents and families regarding medications, treatments, care plans, and healthcare needs.
- Follow infection control, medication safety, patient safety, and facility policies and procedures.
- Prioritize patient care effectively while managing the assigned resident workload.
- Float to other units as required based on operational needs.
- Participate in scheduled every-other-weekend coverage.
- Perform other nursing duties as assigned.
- Minimum 1 year of LVN/LPN nursing experience required.
- Previous long-term care or skilled nursing experience preferred.
- Experience managing a 1:21 resident-to-nurse ratio required.
- Experience caring for residents with dementia, CHF, CVA, arthritis, and other chronic conditions.
- Current BLS certification required.
- Experience using Epic EMR required.
- Strong clinical assessment skills.
- Excellent communication and interpersonal skills.
- Strong organization and time-management abilities.
- Ability to work independently and effectively within an interdisciplinary team.
- Ability and willingness to float between units based on facility needs.
