Job Summary

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Burlington Health & Rehabilitation Center in Burlington, Vermont. As an LPN, you will provide direct nursing care to our residents while supervising the day-to-day nursing activities performed by certified nursing assistants in accordance with current federal, state, and local regulations and facility guidelines.

Responsibilities
  • Direct the daily activities of certified nursing assistants in accordance with current regulations, guidelines, and facility policies

  • Ensure nursing personnel comply with established policies and procedures

  • Participate in resident admissions, discharges, and transfers

  • Observe residents for changes in condition, notify physicians and families or representatives, and document accordingly

  • Transcribe physician orders to medical records and carry out orders as written

  • Prepare and administer medications as ordered and monitor for adverse effects

  • Perform wound treatments as ordered, observe for changes, and document appropriately

  • Schedule follow-up appointments and coordinate transportation needs

  • Provide nursing leadership and guidance to nursing personnel assigned to the unit

  • Provide direct care, including colostomy changes, tube feedings, wound care, suctioning, IV administration, and other skilled nursing interventions as assigned

  • Conduct rounds to ensure resident needs are being met and staff are performing assigned duties

  • Collaborate with interdisciplinary team members to ensure residents' needs are holistically addressed

  • Communicate concerns and complaints to appropriate departments and file grievances as necessary

  • Report incidents and unusual occurrences to appropriate nursing leadership and participate in investigations as required

  • Participate in end-of-life care as needed

  • Ensure adequate stock of medications, supplies, and equipment and notify appropriate personnel of needs

  • Initiate, review, and update resident care plans as required

  • Promote teamwork, mutual respect, and effective communication

  • Participate in facility surveys and inspection processes

Qualifications
  • Graduate of an approved LPN/LVN program

  • Current, unrestricted Vermont LPN license

  • Current CPR certification

  • Prior long-term care experience preferred but not required

  • Strong communication and critical-thinking skills

  • Ability to work effectively as part of a collaborative healthcare team
