Position Overview
We are seeking an experienced Home Health Physical Therapist to provide high-quality, patient-centered care throughout Western Washington. This is an autonomous field-based role that focuses heavily on OASIS assessments, admissions, case management, and skilled therapy services.
The ideal candidate is comfortable working independently, managing a caseload across a designated territory, and coordinating care with physicians and interdisciplinary team members.
Responsibilities
- Complete Start of Care (SOC), Resumption of Care (ROC), recertifications, evaluations, and follow-up visits
- Develop and manage individualized physical therapy plans of care
- Perform OASIS documentation accurately and timely
- Coordinate care with physicians, nurses, social workers, and other healthcare professionals
- Travel within approximately a 50-mile radius to provide home-based patient care
- Maintain compliance with home health regulations and agency standards
Requirements
- Active Washington State Physical Therapist License
- Current BLS Certification
- Valid Driver's License
- Reliable Transportation
- Current Auto Insurance meeting state requirements
- Home Health experience required/preferred
- OASIS experience strongly preferred
- HCHB experience preferred
