Job Description

We are seeking a compassionate and organized Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our AFC and GAFC programs. The nurse will conduct assessments, develop care plans, monitor participant health status, and collaborate with caregivers, case managers, and interdisciplinary teams to ensure high-quality care.

Responsibilities

  • Conduct initial and ongoing nursing assessments.
  • Develop, implement, and update individualized care plans.
  • Complete required AFC and GAFC documentation in compliance with MassHealth regulations.
  • Perform routine participant monitoring and home visits.
  • Educate caregivers and participants on health-related issues.
  • Collaborate with physicians, social workers, case managers, and community providers.
  • Identify changes in participant conditions and recommend appropriate interventions.
  • Maintain accurate and timely electronic documentation.
  • Participate in quality improvement and regulatory compliance activities.

Qualifications

RN Candidates

  • Active Massachusetts Registered Nurse (RN) license.
  • Minimum 1 year of nursing experience preferred.
  • Strong assessment and care management skills.

LPN Candidates

  • Active Massachusetts Licensed Practical Nurse (LPN) license.
  • Minimum 1 year of clinical experience preferred.
  • Experience working with elderly or disabled populations preferred.

Desired Skills

  • AFC, GAFC, home care, case management, or community health experience.
  • Excellent communication and interpersonal skills.
  • Strong organizational and documentation skills.
  • Ability to work independently and manage time effectively.
  • Knowledge of MassHealth programs preferred.
  • Bilingual candidates encouraged to apply.
