Job Summary:
Physical Therapist (PT) specializing in Home Health (HH) services in a home health setting. This role involves providing outpatient therapy to geriatric patients with a focus on post-operative recovery, spinal conditions, and neurological disorders. The position requires flexibility within a designated territory and compliance with all clinical and documentation standards.
Job Details:
- Schedule: 8-hour shifts, Monday through Friday, with a minimum of 40 hours per week
- Required Qualifications: Bachelors or Doctorate degree in Physical Therapy, active state licensure, minimum of 1 year of relevant experience, recent experience with Home Health (HCHB), and OASIS experience required for staff completing Start of Care (SOC). Proficiency in HCHB (Homecare Homebase) required. Must have an active drivers license and reliable transportation.
- Certifications: BLS (Basic Life Support) required
- Additional Information:
- Travel Pairs accepted, with each traveler assigned to their own territory and responsible for own transportation
- All charting must be completed by 9 a.m. the following morning
- On-call rotation may be required; to be discussed during interview
- Timely communication and documentation are mandatory
- All travelers must have a drivers license in hand with submission
- RTO (Requested Time Off) requests must be submitted at the time of application
- Modules are billable and completed during orientation
- Position runs from 09/28/2026 to 12/28/2026
