About the RoleAs a 1915(i) Care Manager / Care Coordinator, you’ll play a vital role in ensuring individuals receive the comprehensive support they need to thrive. This position is perfect for someone with at least one year of experience who is eager to help bridge the gaps between mental health, reentry, family support, education, employment, housing, and more.
Your work will directly contribute to our mission of healing, rebuilding, and connecting individuals to opportunities that foster long-term success and generational progress.
Key ResponsibilitiesAs a Care Manager / Care Coordinator, you will:
- Develop and implement individualized care plans tailored to the unique needs of each client.
- Coordinate services across multiple domains, including mental health, housing, employment, education, and life skills.
- Serve as a central point of contact for clients, ensuring seamless communication and connection to resources.
- Monitor client progress and adjust care plans as needed to ensure continued success.
- Collaborate with community partners and service providers to build a robust support network.
- Advocate for clients, ensuring they have access to the tools and resources necessary to achieve their goals.
To succeed in this role, you’ll need:
- At least 1 year of experience in a related field, such as social work, care coordination, or case management.
- Strong organizational skills and the ability to manage multiple priorities effectively.
- Excellent communication and interpersonal skills to build trust and rapport with clients and community partners.

- The ability to work collaboratively in a dynamic, interconnected environment.
