Responsibilities:

• Provide high-quality patient care, including assistance with daily living activities, vital sign monitoring, and reporting of patient observations

• Maintain a safe and clean environment, adhering to infection control and safety protocols

• Collaborate with healthcare team members to develop and implement individualized care plans

• Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions

Requirements:

• Current certification as a Certified Nursing Assistant (CNA)

• Minimum 0 years of experience in long-term care or post-acute rehabilitation

• Ability to work 6am-6pm shifts, with flexibility to adjust schedule as needed
