Job Summary
Physical Therapist Outpatient (PT OP) Location: Outpatient setting. This position involves providing outpatient physical therapy services with a flexible schedule that includes rotating shifts. The role requires the delivery of quality patient care within an outpatient division, supporting the organizations commitment to health and recovery.
Job Details
- Position Description: Outpatient physical therapy services in an outpatient division, following shift notes of Monday through Friday, 8:00 AM to 4:30 PM, with some flexibility on start and end times.
- Required Qualifications: Active Illinois license or confirmation that the application process has been initiated; BLS Certification; minimum of 1 year of experience; Schedule: Rotating shifts, 8 hours daily, with a minimum of 40 hours per week.
- Preferred Qualifications: None specified.
- Additional Information: Local rate policy applies to clinicians with permanent addresses within 50 miles. The organization has a drug-free campus policy, requiring negative drug screens for all panels. Call-off policy allows for up to twelve hours every thirteen weeks without penalty. Orientation includes 12 hours that are non-billable, with guaranteed hours not applying during orientation. Responsible for charting in EPIC.
- Submission Requirements: Last four digits of clinician's SSN, active IL license or confirmation of application, understanding of drug testing policy, and acknowledgment of the call-off policy.
