MDS Coordinator (Minimum Data Set Coordinator)


Position Summary

The MDS Coordinator is responsible for coordinating and managing the Resident Assessment Instrument (RAI) process, including Minimum Data Set (MDS) assessments, care planning, Medicare and Medicaid reimbursement documentation, and regulatory compliance. This position works closely with the interdisciplinary team to ensure accurate resident assessments, timely submissions, quality outcomes, and optimal reimbursement.

Essential Duties and Responsibilities

MDS & Assessment Management

  • Coordinate and complete all MDS assessments in accordance with CMS, Medicare, Medicaid, and Florida regulations.

  • Maintain the MDS assessment schedule and ensure timely completion of OBRA and PPS assessments.

  • Conduct resident interviews, chart reviews, and clinical assessments.

  • Submit MDS assessments accurately and within required deadlines.

  • Monitor Assessment Reference Dates (ARDs) and ensure compliance with federal guidelines.

Care Planning

  • Develop, implement, and update individualized resident care plans.

  • Coordinate interdisciplinary care plan meetings.

  • Collaborate with nursing, therapy, dietary, social services, activities, and medical staff.

  • Ensure care plans reflect resident needs, goals, and services provided.

Reimbursement & Compliance

  • Ensure documentation supports Medicare and Medicaid reimbursement.

  • Review clinical records for accuracy, completeness, and regulatory compliance.

  • Monitor PDPM coding and reimbursement opportunities.

  • Participate in audits, quality assurance activities, and regulatory surveys.

  • Assist with survey preparation and plan-of-correction activities as needed.

Quality Assurance

  • Conduct routine chart audits.

  • Track quality measures and clinical outcomes.

  • Identify documentation deficiencies and implement corrective actions.

  • Participate in Quality Assurance and Performance Improvement (QAPI) programs.

Education & Leadership

  • Educate nursing staff regarding MDS documentation requirements and regulatory changes.

  • Serve as a resource for MDS, RAI, Medicare, Medicaid, and PDPM questions.

  • Assist with orientation and training of clinical staff.

Qualifications

Required

  • Current Florida Registered Nurse (RN) license preferred; LPN may be accepted based on facility policy.

  • Minimum 1–2 years of skilled nursing or long-term care experience.

  • Experience with MDS 3.0, RAI process, and Medicare/Medicaid regulations.

  • Knowledge of PDPM reimbursement methodology.

  • Proficiency with Electronic Medical Records (EMR).

Preferred

  • RAC-CT certification or willingness to obtain.

  • Previous MDS Coordinator experience in a skilled nursing facility.

  • Experience with Florida AHCA regulations and survey process.
