We’re currently seeking a full-time  Registered Nurse (RN) to perform in-home patient health assessments and provide training support to Home Health Aides (HHAs) and Personal Care Aides (PCAs). This role offers a meaningful way to build connections and make a true impact — one visit at a time.

What You'll Do

• Conduct in-home patient health assessments
• Provide training and support to Home Health Aides (HHAs) and Personal Care Assistants (PCAs)
• Collaborate with a strong clinical support team to ensure the best client outcomes
• Maintain a flexible schedule with manageable weekly productivity expectations
  
  

What You’ll Need

• Active RN license in New York, in good standing
• Ability to work independently in the field
• Comfortable traveling to 2 out of 4 boroughs, Queens, Brooklyn, Staten Island, and NYC
• Bilingual in Chinese or Spanish a huge plus
• Home care experience a plus
• HHAeXchange knowledge a plus
