Social Services Designee – Mason Health Care Center | Cincinnati, OH

Mason Health Care Center, a small, boutique-style skilled nursing facility, is seeking a dedicated Social Services Designee to support our residents and families. This is a great opportunity to make a meaningful impact in a close-knit environment where residents’ well-being truly comes first.

What You’ll Do:

• Coordinate and assist with admission, care, and discharge planning for residents.

• Provide emotional support, resources, and guidance to residents and families.

• Maintain accurate documentation and ensure compliance with facility, state, and federal requirements.

• Work closely with nursing, therapy, and interdisciplinary teams to ensure smooth transitions and quality care.

• Foster a positive environment that promotes residents’ well-being, adjustment, and quality of life.

What We’re Looking For:

• Experience in a skilled nursing or long-term care setting preferred.

• Strong communication, organizational, and teamwork skills.

• Compassionate, resident-focused, and detail-oriented individual.

• LSW license not required — we will train the right candidate with a passion for working with seniors.
