The Patient Care Manager is responsible for the coordination of all clinical services (Planning, organizing, directing and supervising) provided to the hospice patient. The Patient Care Manager is a member of the interdisciplinary team. This position oversees the day to day clinical operation of hospice in conformance with the organization's mission, philosophy and goals and ensures that services conform to all applicable laws, rules and regulations.

Reports to: Program Director

Qualifications:

• Licensed as a Registered Nurse in PA.

• Education: A Registered Nurse with a Baccalaureate or higher degree preferred in nursing or other health related field. And two year’s clinical experience with the last three (3) years in a home health agency, home care setting or health facility.

• Experience: Two (2) to four (4) years’ experience in a patient care setting, acute care desirable OR two (2) to four (4) years’ experience in a supervisory or administrative position in a hospice organization OR equivalent and demonstrates a strong knowledge of regulations

• Monitors patient care activities to ensure competent and compassionate care is provided to patients.

• Must possess strong patient assessment, patient management and documentation skills
