Physical Therapist Outpatient Fort Collins, Colorado. Job Details:
- Schedule: Days shift, 8 hours per day, with a minimum of 40 hours per week
- Required Qualifications: Degree in Physical Therapy from an accredited school; 6 months to 1+ year of outpatient clinical experience; CPR certification; active and applicable license for the state of Colorado; successful completion of a previous assignment with the agency or the organization (highly desired)
- Additional Information: Completes comprehensive patient evaluations, creates and modifies treatment plans, and documents medical records accurately and timely. Participates in student education with supervision and provides a positive learning environment. Maintains licensure in good standing and follows all state practice acts and regulatory guidelines. Demonstrates commitment to performance improvement initiatives and organizational safety protocols.
- Float: Float rotation may be required.
