Medical Records Clerk

We are looking for a detail-oriented Medical Records Clerk to support documentation review and provider outreach for a growing healthcare team in Centennial, Colorado. This Contract to Permanent position is ideal for someone who can evaluate clinical records thoughtfully, manage follow-up communication with medical offices, and stay effective in a changing environment. The role requires strong judgment, professionalism, and confidence working across electronic systems while helping ensure records meet established guidelines.

Responsibilities:

• Examine patient charts, progress notes, and supporting medical documentation to confirm completeness, accuracy, and alignment with required standards.

• Process certificates and related records by reviewing details carefully and making informed decisions based on clinical documentation.

• Place outbound calls to physicians' offices and other healthcare providers to obtain missing records, verify documentation status, and follow up on outstanding items.

• Respond to incoming calls professionally and assist with questions related to medical documentation and record processing.

• Navigate multiple electronic systems throughout the day, including newer tools that support document review, and apply independent judgment when validating flagged information.

• Assess records highlighted by automated review technology and determine whether the documentation supports qualification criteria.

• Maintain organized documentation workflows and update records consistently to support timely processing.

• Participate in weekly team meetings to share updates, discuss case progress, and stay aligned on priorities.

• At least 2 years of relevant experience, with 3 to 4 years in a medical or clinical documentation environment strongly preferred.

• Hands-on experience reviewing medical records, charts, or related healthcare documentation in an electronic medical record system.

• Working knowledge of diagnosis codes and other medical coding terminology used in clinical records.

• Ability to think critically, investigate documentation thoroughly, and make sound decisions based on record content.

• Detail-oriented communication skills with the ability to build productive relationships with healthcare providers and office staff.

• Comfort handling outbound provider calls and managing documentation follow-up in a fast-paced setting.

• Adaptability and willingness to learn new systems, tools, and processes as the role evolves.

• Experience with durable medical equipment is a plus.
