Occupational Therapist

Position Overview:
We are seeking an Occupational Therapist to provide patient-centered rehabilitative care in a skilled nursing environment.

Key Responsibilities:

• Perform OT evaluations and reassessments

• Develop and implement individualized care plans

• Address ADLs, mobility, and safety needs

• Participate in interdisciplinary care planning

• Complete clinical documentation accurately and timely

Qualifications:

• Active CA OT license

• SNF experience preferred
