About the Role:
Aura One Hospice is looking for a compassionate and dedicated Certified Home Health Aide (CHHA) to join our team in Palm Desert, CA. Responsibilities:

• Provide personal care services to hospice patients in the home setting, including bathing, grooming, and hygiene assistance
• Assist patients with mobility, transfers, and ambulation as directed by the care plan
• Monitor and document patient condition changes and report observations to the supervising nurse
• Support patients with light housekeeping, meal preparation, and activities of daily living (ADLs)
• Deliver compassionate, patient-centered care in alignment with hospice philosophy and end-of-life care standards
• Communicate effectively with interdisciplinary hospice team members, patients, and families
• Maintain accurate documentation and adhere to care plan guidelines

Requirements:

• Valid California Certified Home Health Aide (CHHA) certification required
• Prior experience in hospice, home health, or palliative care preferred
• Current CPR/First Aid certification
• Reliable transportation and valid driver's license
• Strong interpersonal skills with a compassionate, patient-focused demeanor
• Ability to work independently and as part of a collaborative care team
• Comfortable working in home environments with patients facing end-of-life care needs
