We are currently accepting applications for a Family Support Facilitator to help support children, youth, and families as they navigate challenges and connect with the services and resources they need to thrive.

About the Position

The Family Support Facilitator provides strength-based, family-driven, and culturally responsive services to at-risk youth and families identified through local school systems and other community partners. This position plays an important role in helping children and families access support, understand available resources, and build connections within their schools and communities. As a Family Support Facilitator, you will work collaboratively with families, schools, and community agencies to identify needs, coordinate services, and help ensure families are informed and supported throughout the process.

What You'll Do

Key responsibilities include:

  • Providing strength-based, family-driven, and culturally responsive support to children, youth, and families
  • Building positive relationships with families, school personnel, and community partners
  • Consulting and collaborating with schools and community agencies to identify and support family needs
  • Accepting referrals and completing intake and enrollment documentation
  • Assessing family strengths, needs, and priorities
  • Helping families understand available services, resources, and support options
  • Connecting children and families with appropriate community resources and services
  • Supporting families as they navigate systems and access the assistance they need
  • Promoting collaboration between families, schools, service providers, and other community stakeholders
  • Maintaining appropriate and timely documentation in accordance with agency requirements
  • Contributing to a positive, collaborative, and supportive team environment

Education & Experience

Candidates must have a minimum of a high school diploma or equivalent.
