Physical Therapist
Job Summary: The Physical Therapist will provide essential therapy services to patients in their residences, addressing physical impairments, functional limitations, or disabilities arising from various causes such as injury or disease. This role encompasses a diverse patient demographic, ranging from newborns to elderly individuals. Collaboration with patients, families, physicians, and other allied health professionals is essential to identify patient needs and coordinate appropriate services. The Physical Therapist will maintain comprehensive and timely electronic documentation and engage in interdisciplinary communications in alignment with agency policies and standards of practice.
Qualifications:- Current Physical Therapist Professional license in Massachusetts.
- Minimum of 1 year of travel experience as a Physical Therapist.
- Minimum of 1 year of experience in home care.
- Ability to travel to various cases as assigned.
- Deliver quality physical therapy services to patients in their own homes.
- Assess patient needs and develop customized treatment plans.
- Work collaboratively with patients, families, physicians, and other healthcare providers.
- Document patient progress and modify treatment plans as necessary.
- Adhere to agency policies and procedures while providing care.
This position will cover areas in the North Region, including Arlington, Everett, Malden, Medford, West Medford, Melrose, and Somerville.
