Essential Functions: As a Registered Nurse (RN) in Home Health, you will:

  • Provide and direct nursing care to patients at home in compliance with applicable laws, regulations, and agency policies.
  • Coordinate the total plan of care with other healthcare professionals.
  • Maintain continuity of patient care by planning and exchanging information with physicians, agency personnel, patients, families, and community resources within a Point of Care setting.

Responsibilities:

  • Provide clinical services within the scope of practice defined by state laws.
  • Conduct initial and comprehensive nursing evaluation visits, determine the primary focus of care, and develop the plan of care within state-specific guidelines.
  • Regularly evaluate and re-evaluate the patient's nursing needs.
  • Ensure patients meet home health eligibility and medical necessity guidelines as defined by payer sources.
  • Initiate, develop, implement, and revise the plan of care in collaboration with physicians and other healthcare professionals involved in care.

License Requirements:

  • Current driver's license and vehicle insurance, with access to a dependable vehicle or public transportation.
  • Current CPR certification.
  • Current RN License in the state of practice.
