RN Case Manager - OPWDD

WCS Healthcare Partners is seeking a full-time Registered Nurse Case Manager to provide clinical oversight, case management, and patient care to individuals with Intellectual and Developmental Disabilities (I/DD) throughout Dutchess, Ulster, Columbia, and Westchester counties. This role combines clinical nursing expertise with individualized care planning in a supportive, mission-driven environment. New graduate RNs are welcome to apply!

Position Details

• Location: Dutchess, Ulster, Columbia, and Westchester counties, NY
• Schedule: Full-time, day shift
• Start Time: Flexible start time of 7:00 AM, 8:00 AM, or 9:00 AM
• Weekends: No weekends
• On-Call: No on-call requirements
• Population: Adults with moderate to profound I/DD

Key Responsibilities

• Manage an OPWDD caseload of approximately 18–25 individuals, ages 20 through adulthood.
• Travel between assigned residential group homes throughout the region to provide nursing oversight and support.
• Conduct health assessments and monitor individuals' ongoing medical and nursing needs.
• Develop and maintain individualized health and care plans.
• Maintain accurate health records and ensure compliance with applicable developmental disabilities nursing standards.
• Oversee and support medication administration by AMAP-certified staff.
• Attend medical appointments as needed and collaborate with physicians, healthcare providers, and other members of the care team.
• Participate in interdisciplinary team meetings and individualized planning.
• Provide education, guidance, and clinical support to residential staff.

Minimum Qualifications

• Current New York State RN license.
• Strong communication, collaboration, and teamwork skills.
• Compassion for individuals with I/DD and a commitment to providing high-quality, person-centered care.
• Ability and willingness to travel between assigned residential group homes throughout the assigned counties.
• Strong interest in collaborative nursing and individualized patient care.
