WCS Healthcare Partners is seeking a full-time Registered Nurse Case Manager to provide clinical oversight, case management, and patient care to individuals with Intellectual and Developmental Disabilities (I/DD) throughout Dutchess, Ulster, Columbia, and Westchester counties. This role combines clinical nursing expertise with individualized care planning in a supportive, mission-driven environment. New graduate RNs are welcome to apply!
Position Details
- Location: Dutchess, Ulster, Columbia, and Westchester counties, NY
- Schedule: Full-time, day shift
- Start Time: Flexible start time of 7:00 AM, 8:00 AM, or 9:00 AM
- Weekends: No weekends
- On-Call: No on-call requirements
- Population: Adults with moderate to profound I/DD
Key Responsibilities
- Manage an OPWDD caseload of approximately 18–25 individuals, ages 20 through adulthood.
- Travel between assigned residential group homes throughout the region to provide nursing oversight and support.
- Conduct health assessments and monitor individuals' ongoing medical and nursing needs.
- Develop and maintain individualized health and care plans.
- Maintain accurate health records and ensure compliance with applicable developmental disabilities nursing standards.
- Oversee and support medication administration by AMAP-certified staff.
- Attend medical appointments as needed and collaborate with physicians, healthcare providers, and other members of the care team.
- Participate in interdisciplinary team meetings and individualized planning.
- Provide education, guidance, and clinical support to residential staff.
Minimum Qualifications
- Current New York State RN license.
- Strong communication, collaboration, and teamwork skills.
- Compassion for individuals with I/DD and a commitment to providing high-quality, person-centered care.
- Ability and willingness to travel between assigned residential group homes throughout the assigned counties.
- Strong interest in collaborative nursing and individualized patient care.
