Outpatient Physical Therapist – Travel Contract

• Start: Mid-September
• Setting: Outpatient
• Evaluations: 60 minutes
• Treatments/Follow-ups: 45 minutes
• Evaluate patients and develop individualized plans of care
• Establish functional goals and modify treatment plans based on patient progress
• Provide hands-on treatment focused on improving function and return to daily/community activities
• Educate patients and families on treatment plans, exercises and progression
• Complete accurate and timely documentation, including evaluations, goals and progress
• Coordinate care with physicians and other members of the clinical team
• Appropriately delegate and supervise therapy services as needed
• Maintain patient and employee safety standards
• Follow applicable state practice acts, professional standards and regulatory requirements
• Maintain active PT licensure and required continuing education
• Participate in quality improvement and professional development activities
