Occupational Therapist - Home Care

We are seeking a compassionate and highly experienced Occupational Therapist to provide exceptional, patient-centered care in a home-based setting. This role is ideal for a clinician who combines clinical expertise with genuine empathy, and who is passionate about helping patients regain independence, restore daily function, and improve their overall quality of life within the comfort of their own homes.

Responsibilities:

• Conduct comprehensive in-home assessments of patients’ functional abilities and living environments
• Develop and implement personalized, goal-oriented treatment plans
• Provide skilled occupational therapy services focused on activities of daily living (ADLs) and functional independence
• Recommend and train patients in the use of adaptive equipment and home modifications
• Educate patients and caregivers to promote safety, independence, and long-term success
• Monitor patient progress and adjust treatment plans accordingly
• Maintain accurate, timely, and compliant documentation
• Collaborate with physicians and interdisciplinary team members to ensure coordinated care

Qualifications:

• Must be a Licensed Occupational Therapist (OT) in Michigan
• Extensive experience in home health or community-based care
• Strong clinical reasoning and problem-solving skills
• Ability to work independently and manage a flexible caseload
• Excellent communication and interpersonal skills
• Compassionate, patient-focused approach to care
• Reliable transportation and willingness to travel locally
