The CNA / HHA will provide essential patient care services primarily in a home health care setting. This role involves assisting 1-3 patients daily with basic daily living activities, requiring travel between multiple patient locations. The ideal candidate will deliver compassionate support to enhance patients' quality of life.
Responsibilities
- Provide basic daily living assistance to patients
- Travel daily between 1-3 patient homes for care provision
- Monitor patient health and report changes to healthcare team
- Support patients with personal hygiene and mobility
- Maintain accurate patient care records
Preferred Qualifications
- 1+ years experience in patient care
- Certified CNA or CHHA
- Certified Nursing Assistant (CNA) License
- HHA Certification
- Basic Life Support (BLS) certification
