Service Area: Monroe to Taylor, Michigan
Are you a compassionate, dependable, and skilled Occupational Therapist (OT) who enjoys helping patients regain independence and safely participate in the activities that matter most to them?
We are looking for a motivated and patient-focused OT to join our growing home health team. This is a rewarding opportunity for an OT who enjoys one-on-one patient care, clinical independence, flexible scheduling, and seeing the direct impact of their work.
What You’ll DoAs a Home Health OT, you will provide individualized occupational therapy services in patients’ homes, helping them improve function, safety, independence, and quality of life.
Your responsibilities will include:
Completing comprehensive occupational therapy evaluations and assessments in the patient's home.
Assessing patients' ability to safely perform activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
Developing individualized treatment plans based on each patient's functional needs, goals, and physician orders.
Providing skilled occupational therapy treatments and interventions according to the plan of care.
Helping patients improve strength, coordination, balance, mobility, endurance, and functional abilities as appropriate to their occupational therapy needs.
Providing training in dressing, bathing, grooming, toileting, transfers, meal preparation, and other daily activities as appropriate.
Assessing the home environment and recommending appropriate safety modifications, adaptive techniques, and durable medical equipment (DME) when indicated.
Educating patients and caregivers on strategies to promote independence and prevent injury or decline.
Monitoring patient progress and modifying treatment approaches as clinically appropriate.
Collaborating with RNs, LPNs, PTs, physicians, caregivers, and other members of the interdisciplinary team.
Communicating changes in patient condition, progress, barriers, and recommendations to the appropriate members of the care team.
Completing accurate, timely, and compliant clinical documentation.
Helping patients work toward meaningful functional goals that allow them to remain safely and independently in their homes.
This position will primarily serve patients throughout the Monroe to Taylor area.
Reliable transportation and the ability to travel between patient homes are required.

Compensation:$55 - $75 hourly
Responsibilities:
- Develop and implement personalized care plans tailored to each patient's unique needs and goals.
- Collaborate with an interdisciplinary team to ensure comprehensive and coordinated patient care.
- Conduct thorough assessments to evaluate patients' physical, emotional, and social needs.
- Provide hands-on therapy to enhance patients' daily living skills and promote independence.
- Educate patients and their families on therapeutic techniques and strategies for continued progress.
- Maintain accurate and timely documentation of patient progress and therapy sessions.
- Adapt therapy interventions based on patient feedback and evolving needs to ensure optimal outcomes.
Qualifications:
- Experience in conducting comprehensive patient evaluations, focusing on activities of daily living, mobility, and cognitive function.
- Ability to develop and implement personalized treatment plans that align with patient goals and physician directives.
- Proven track record of delivering skilled occupational therapy interventions to enhance patient independence and safety.
- Familiarity with conducting home safety assessments and recommending appropriate adaptive equipment.
- Strong communication skills to effectively educate patients and caregivers on disease management and assistive device usage.
- Proficiency in completing required documentation, including OASIS assessments and progress notes, accurately and promptly.
- Experience in collaborating with interdisciplinary teams to ensure cohesive patient care and successful outcomes.
