• Provide emotional support, counseling, and education to patients and their families
• Coordinate and facilitate patient-centered care transitions, including discharge planning and follow-up care
• Develop and maintain relationships with patients, families, and community resources to ensure continuity of care
• Document all patient interactions, assessments, and care plans accurately and timely
