RN Registered Nurse Case Manager for Home Health
Job Summary:

RN Registered Nurse Case Manager Responsibilities:
Assess patients' physical and emotional needs in their homes and develop individualized care plans to ensure optimal health outcomes
Coordinate and collaborate with patients, families, caregivers, and healthcare providers to ensure seamless continuity of care
Provide skilled nursing care, including wound care, medication management, and education to patients and caregivers
Conduct regular home visits to monitor patients' progress, identify potential breakthroughs, and make adjustments to care plans as needed
Communicate effectively with patients, families, and healthcare providers to ensure timely and accurate transmission of patient information
Develop and maintain accurate and up-to-date patient records and reports
Participate in quality improvement initiatives to improve patient outcomes and satisfaction
Collaborate with interdisciplinary teams to develop and implement patient-centered care plans
RN Registered Nurse Case Manager Requirements:
Current licensure as a Registered Nurse in the State of Pennsylvania
Prior experience in home health care, case management, or a related field
Strong clinical and case management skills, with the ability to assess patient needs and develop effective care plans
Excellent communication, interpersonal, and organizational skills
Ability to work independently and as part of a team
Possess a valid driver's license and reliable transportation
