About the Role:
HomeWell Care Services CA305 is looking for a passionate and dedicated Care Manager to join our growing team in Upland, CA, serving San Bernardino County. In this rewarding role, you'll be at the heart of connecting clients with the compassionate care they deserve, making a real difference in the lives of individuals and families every day.

Responsibilities:

• Conduct client assessments and develop individualized care plans tailored to each client's needs
• Coordinate and oversee in-home care services for clients throughout San Bernardino County
• Serve as the primary point of contact for clients, families, and caregivers to ensure quality care delivery
• Monitor and evaluate client progress, adjusting care plans as needs evolve
• Supervise caregiving staff to maintain high service standards
• Build and maintain relationships with referral sources, healthcare providers, and community partners
• Ensure compliance with state regulations, company policies, and care standards

Requirements:

• Experience in home care, case management, or a related healthcare field
• Strong knowledge of in-home care services and San Bernardino County resources
• Excellent communication, organizational, and interpersonal skills
• Ability to manage multiple clients and priorities in a fast-paced environment
• Compassionate, client-centered approach to care coordination
• Valid California driver's license and reliable transportation for county-wide travel
• Experience with caregiver supervision and staff management preferred
