As a MDS Coordinator at Caring Acres Nursing and Rehab Center, you will play a vital role in ensuring the highest level of care for our residents. You will be responsible for coordinating and implementing individualized care plans, gathering and analyzing resident data, and collaborating with our multidisciplinary team to provide comprehensive care. Responsibilities:

• Coordinate and implement individualized care plans for residents

• Gather and analyze resident data to ensure accurate and effective care

• Collaborate with our multidisciplinary team to provide comprehensive care

• Participate in care conferences and-family meetings

• Develop and maintain accurate and up-to-date resident records

• Ensure compliance with regulatory requirements and industry standards

• Provide exceptional customer service to residents, families, and staff

Requirements:

• 2 years of long-term care nursing experience (minimum)

• Education: State of Iowa RN Licensure
