Serenity Hospice PA is seeking a compassionate and dedicated Hospice RN Case Manager (RNCM) to join our growing team serving patients and families throughout Philadelphia, PA.

Your Role

As a Hospice RN Case Manager, you will play a key role in coordinating and providing exceptional end-of-life care. You will be the trusted clinical presence for your patients and families while collaborating closely with physicians, caregivers, and the hospice interdisciplinary team.

Responsibilities

  • Manage and coordinate individualized hospice patient care plans

  • Perform comprehensive nursing assessments and ongoing patient evaluations

  • Provide expert symptom management and compassionate nursing care

  • Educate patients and families regarding disease progression, medications, and comfort-focused care

  • Collaborate with physicians, caregivers, and interdisciplinary team members

  • Maintain accurate, timely, and thorough clinical documentation

  • Provide emotional support and guidance to patients and their loved ones

  • Advocate for patient comfort, dignity, and quality of life

What We’re Looking For

  • Active Pennsylvania RN license

  • Strong clinical assessment and organizational skills

  • Excellent communication and interpersonal abilities

  • A compassionate, patient-centered approach to care

  • Ability to work independently while being an engaged member of a team

  • Valid driver’s license and reliable transportation
