Registered Nurse - LTC - South Mountain

WCS Healthcare Partners is seeking a Registered Nurse (RN) to provide comprehensive nursing care within a state-operated Veterans Long Term Care facility in South Mountain, Pennsylvania. This role offers the opportunity to support honorably discharged Veterans with chronic medical conditions, physical disabilities, cognitive impairment, behavioral health needs, and age-related care requirements.

Schedule:

• Evening Shift: 3:00 PM – 11:00 PM
• Night Shift: 11:00 PM – 7:00 AM
• 8-hour shifts available

Job Responsibilities:

• Provide comprehensive nursing care to Veterans in a long-term care setting.
• Assess, monitor, and document patient health status and changes in condition.
• Administer medications and treatments in accordance with physician orders and established nursing standards.
• Develop and implement individualized care plans in collaboration with the interdisciplinary care team.
• Respond to changes in patient condition and coordinate appropriate interventions.
• Provide patient and family education regarding care plans, treatments, and health conditions.
• Maintain accurate and timely clinical documentation.
• Ensure compliance with facility policies, state regulations, and professional nursing standards.

Minimum Qualifications:

• Current Registered Nurse (RN) license issued by the Pennsylvania State Board of Nursing, or a valid non-renewable temporary practice permit.
• AHA BLS certification required.
• At least 2 years of nursing experience preferred.
• Previous long-term care, geriatric, rehabilitation, or Veterans healthcare experience preferred.
