Senior 245D Designated Coordinator / Designated Manager
Northeastern Home Group LLC | Edina, MN | Full-Time
Northeastern Home Group LLC is seeking an experienced, dependable, highly organized 245D Designated Coordinator / Designated Manager to lead client intake, service planning, client documentation, ongoing reviews, and coordination between clients, case managers, support teams, and direct-support staff.
We are looking for an experienced 245D professional — not someone who is learning 245D for the first time.
The ideal candidate understands Minnesota Chapter 245D, person-centered planning, support plans, Support Plan Addendums, Individual Abuse Prevention Plans (IAPPs), applicable assessments, service outcomes, client-specific documentation, required reviews, case-manager communication, and the importance of completing every requirement accurately and on time.
This position requires someone who takes ownership, follows through, communicates well, works collaboratively, shows up consistently, and keeps client records organized and audit-ready.
What You Will Own
Client Intake & Service Initiation
- Respond promptly to new referrals.
- Review referral information and identify what is needed before services begin.
- Request applicable support plans, authorizations, assessments, and other necessary information from case managers/care coordinators.
- Communicate professionally with clients, legal representatives, case managers, and support-team members.
- Review support plans to understand authorized services, assessed needs, risks, preferences, outcomes, and service expectations.
- Identify missing, inconsistent, or incomplete information and follow through until it is resolved.
- Coordinate with staffing and leadership regarding new service starts.
245D Service Planning & Documentation
- Develop and maintain accurate, individualized, person-centered client documentation.
- Develop and update applicable Support Plan Addendums.
- Develop and update Individual Abuse Prevention Plans (IAPPs).
- Complete or coordinate applicable self-management, health/medical, safety, and other required assessments.
- Develop measurable person-centered service outcomes and supports.
- Ensure provider documentation accurately reflects the person's support plan, assessed needs, preferences, risks, authorized services, and actual service delivery.
- Update documents following meetings, reassessments, changes in needs, or changes to the person's support plan.
- Obtain and track required signatures and dates.
Deadlines, Reviews & Compliance
- Maintain a compliance calendar for assigned clients.
- Track applicable service-initiation deadlines.
- Coordinate required initial planning meetings and assessments.
- Track applicable 45/60-day requirements.
- Track semiannual/six-month reviews when required by the person's plan or applicable requirements.
- Track annual assessments and reviews.
- Schedule requirements before deadlines become urgent.
- Follow up on missing documents and signatures.
- Verify completion instead of simply marking tasks complete.
- Escalate overdue, missing, inconsistent, or high-risk items to leadership.
Case Manager & Support Team Relationships
- Build strong, professional relationships with case managers and care coordinators.
- Schedule, prepare for, and participate in client planning and review meetings.
- Provide appropriate updates regarding services, progress, outcomes, concerns, and changes.
- Respond promptly to case-manager requests.
- Document decisions and action items from meetings.
- Complete required revisions after meetings.
- Obtain signatures and distribute finalized information appropriately.
- Help make Northeastern Home Group a responsive and dependable provider for clients and their support teams.
Connecting the Plan to Direct Service Staff
- Translate client plans into clear service-delivery expectations.
- Work collaboratively with staffing leadership and direct-support teams.
- Communicate client-specific needs, outcomes, risks, preferences, and service instructions.
- Help identify client-specific staff training and competency needs.
- Provide instruction and assistance to staff implementing support plans and outcomes within the responsibilities of the position.
- Monitor whether services are being implemented consistently with the person's plan.
- Communicate changes promptly to appropriate team members.
- Identify service-delivery concerns and escalate them before they become larger compliance or quality problems.
Documentation Ownership
We expect this position to own the entire documentation cycle:
REVIEW → DEVELOP → MEET → UPDATE → OBTAIN SIGNATURES → UPLOAD/FILE → VERIFY → TRACK NEXT DEADLINE
A document is not considered complete simply because it was drafted.
The successful candidate follows through until applicable meetings, revisions, signatures, uploads, filing, communication, and follow-up requirements are completed.

- Responds promptly to referrals, emails, and follow-up requests.
- Can manage multiple client deadlines simultaneously.
- Is strong with signatures, dates, document versions, electronic records, scanning, uploading, and filing.
- Works independently without needing constant reminders.
- Communicates clearly and professionally.
- Works well with different personalities and departments.
- Is a strong team player and emerging or experienced leader.

Preferred Experience
Strong preference will be given to candidates with previous experience as a:
- 245D Designated Coordinator
- 245D Designated Manager
- HCBS Program Coordinator
- HCBS Program Manager
- Client Services Manager/Coordinator
- 245D Compliance Coordinator
- Case Manager or Care Coordinator with strong provider-side 245D experience
- Human Services Supervisor or similar HCBS leadership professional
Qualifications
Candidates assigned Designated Coordinator and/or Designated Manager responsibilities must meet applicable Minnesota qualification requirements for the position.
Northeastern Home Group will verify education, experience, training, and other qualification documentation before assigning duties requiring those qualifications.
