Licensed Assisted Living Director – 144G & 24-Hour Customized Living

Northeastern Home Group LLC is seeking an experienced, highly organized, hands-on Licensed Assisted Living Director (LALD) to lead our Minnesota Assisted Living and 24-Hour Customized Living operations.

This is not an entry-level Assisted Living leadership position. We are looking for an experienced Minnesota Assisted Living professional who already has strong working knowledge of Minnesota Chapter 144G, Assisted Living operations, Customized Living/24-Hour Customized Living, regulatory compliance, resident services, staffing, training, documentation, quality management, and survey readiness.

The successful candidate will take ownership of the overall operation and work closely with ownership, nursing, staffing leadership, DSPs, residents, case managers/care coordinators, and other members of each resident's support team.

Key Responsibilities

144G Compliance & Survey Readiness

  • Maintain strong working knowledge of Minnesota Chapter 144G requirements.
  • Maintain the facility in a survey-ready condition.
  • Conduct routine compliance audits and facility rounds.
  • Monitor resident rights, contracts, assessments, service plans, records, staffing, training, nursing requirements, medication-management systems, infection control, emergency preparedness, complaints, incidents, quality management, and applicable physical-environment requirements.
  • Identify compliance gaps before they become deficiencies.
  • Assign corrective actions, deadlines, and responsible parties and verify completion.
  • Maintain organized evidence demonstrating compliance.

24-Hour Customized Living

  • Oversee applicable Customized Living and 24-Hour Customized Living requirements.
  • Ensure current support plans and applicable service information are obtained and reviewed.
  • Maintain communication with case managers/care coordinators.
  • Monitor service authorizations and changes affecting service delivery.
  • Ensure services are person-centered and consistent with applicable plans and assessed needs.
  • Coordinate changes with residents, support teams, nursing, staffing, and direct-support personnel.
  • Escalate authorization, planning, staffing, or service-delivery discrepancies.

Resident Services

  • Maintain visibility into the needs and services of every resident.
  • Ensure residents receive the services they are supposed to receive.
  • Monitor resident safety, rights, satisfaction, concerns, and changes in condition.
  • Participate in applicable support-team meetings.
  • Maintain strong communication with residents, representatives, and case managers.
  • Ensure updated support plans and other necessary information are requested and obtained.
  • Follow through on resident concerns until resolution.
  • Immediately escalate health, safety, medication, maltreatment, rights, or service-delivery concerns through the appropriate process.

Staffing & Scheduling

  • Maintain operational oversight of staffing schedules and coverage.
  • Work proactively with staffing leadership to ensure shifts are covered.
  • Identify staffing shortages and open shifts early.
  • Monitor call-offs, attendance, overtime, staffing gaps, and recurring scheduling problems.
  • Coordinate recruiting and hiring needs.
  • Ensure staffing assignments appropriately support resident needs.
  • Hold employees accountable for attendance, professionalism, service quality, and performance.

Staff Training & Competency

  • Monitor staff onboarding and applicable training requirements.
  • Ensure staff receive appropriate resident-specific orientation before performing assigned duties.
  • Monitor required initial and ongoing training.
  • Coordinate with the RN regarding applicable nursing delegation, medication training, and competency requirements.
  • Monitor background-study and employee compliance requirements.
  • Identify expired, incomplete, or missing requirements and take corrective action.
  • Ensure staff understand the resident plans and expectations relevant to the services they provide.

Nursing & Clinical Oversight

  • Work closely with the Registered Nurse to maintain visibility into clinical requirements and deadlines.
  • Monitor systems for assessments, reassessments, service-plan updates, medication management, delegation, competency, changes in condition, provider orders, and required follow-up.
  • Hold responsible team members accountable for timely completion.
  • Escalate overdue or high-risk clinical items.

The Assisted Living Director does not replace the RN or independently perform duties requiring a nursing license.

Physical Environment & Facility Operations

  • Conduct regular facility rounds.
  • Monitor cleanliness, organization, sanitation, food safety, maintenance, emergency preparedness, life-safety systems, required inspections, medication-storage areas, secure records, resident areas, staff areas, and environmental hazards.
  • Monitor applicable fire-safety equipment and inspection/maintenance requirements.
  • Ensure maintenance concerns are documented, assigned, corrected, and verified.
  • Maintain professional, organized electronic and physical record systems.

Documentation & Deadline Management

  • Maintain compliance trackers, calendars, spreadsheets, and dashboards.
  • Monitor assessments, service plans, support plans, authorizations, reviews, signatures, staff requirements, clinical requirements, inspections, and other applicable deadlines.
  • Ensure current documents are available to the appropriate team members.
  • Follow up on missing signatures and documentation.
  • Maintain organized electronic and applicable physical files.
  • Conduct routine record audits.

The expectation is simple: a task is not complete until completion can be verified.

Leadership & Accountability

  • Provide strong day-to-day leadership.
  • Maintain a consistent presence in the operation.
  • Work collaboratively with nursing, staffing, client-services personnel, DSPs, and ownership.
  • Set clear expectations and follow up.
  • Build a professional, respectful, accountable workplace.
  • Identify problems early and develop solutions.
  • Hold team members accountable while supporting their success.
  • Report operational risks and significant concerns promptly to ownership.

The Director will provide leadership with clear visibility into:

We do not want surprises. Problems should be identified early, communicated, assigned, corrected, and verified.

Qualifications

  • Active Minnesota Licensed Assisted Living Director (LALD) strongly preferred.
  • Significant Minnesota Assisted Living leadership experience.
  • Strong working knowledge of Minnesota Chapter 144G.
  • Hands-on Customized Living/24-Hour Customized Living experience strongly preferred.
  • Experience preparing for or participating in MDH Assisted Living surveys strongly preferred.
  • Knowledge of resident rights and person-centered service delivery.
  • Experience managing staff, schedules, training, performance, and accountability.
  • Experience collaborating with Registered Nurses and monitoring clinical compliance systems.
  • Experience communicating with case managers/care coordinators and support teams.
  • Strong documentation and regulatory-record management skills.
  • Strong computer skills, including Excel/spreadsheets and electronic record systems.
  • Excellent organization, communication, leadership, and follow-through.
  • Ability to manage multiple priorities and deadlines without losing accountability.
