- Educate patients and caregivers on exercises, mobility, and safe transfers.
- Collaborate with nursing staff, physicians, and the interdisciplinary care team.

Requirements
- Active New Hampshire Physical Therapist license or eligibility to obtain.
- Recent PT experience preferred.
- SNF experience preferred.
- Current BLS certification.
- Strong communication, clinical reasoning, and patient care skills.
