Description
American Traveler is seeking a Physical Therapist for a Home Health assignment requiring 1+ year of recent home health experience and HCHB EMR proficiency.
Details
- Home Health setting covering the Anacortes, Oak Harbor, and Coupeville territory
- Day shift schedule, 8am-5pm, Monday-Friday, 5x8 hour shifts
- No weekend or on-call requirements
- Requires in-office visits at least once every one to two weeks for team meetings, supplies, and admission packets
- Caseload of approximately 5-7 patients per day with 6 visits daily and a minimum of 30 productivity points weekly
- Patient population primarily geriatric
- Territory extends up to a 60-mile radius from the main office and may require ferry travel
- Uses HCHB EMR system
- iPad provided for documentation; personal cell phone not required
- Mileage reimbursed at the IRS standard rate
Requirements
- Minimum 1 year of recent Home Health experience required
- BLS certification required
- OASIS experience required
- HCHB EMR experience required
- License required if applicable
- Experience with orthopedic, neurologic, and post-operative patient populations preferred
- Skilled in home exercise program development and patient/family education
- Must be flexible with territory assignments
- Timely documentation and communication required
Additional Information
- Responsibilities include home-based patient assessment, treatment planning, and rehabilitation for geriatric patients
- Scrubs required in any color; no jeans permitted
- Local candidates accepted at the travel rate
