Position Type: Full-Time | Hybrid (Office / Local Travel Required)
We are seeking a dedicated and organized Field Care Coordinator to join our home care team. In this role, you will serve as the primary bridge between our agency, consumers, and Personal Care Attendants (PCAs). You will oversee the entire lifecycle of consumer care—from initial software data entry and EVV clock-in training to monthly check-in calls, annual in-person home visits, and day-to-day scheduling coordination.
Key Responsibilities
Consumer & PCA Onboarding
- Input new consumer and caregiver/PCA profiles into the agency software and EVV systems accurately.
- Conduct comprehensive Welcome Calls for new participants to introduce services and establish care expectations.
- Educate and train consumers and PCAs on Electronic Visit Verification (EVV) software, teaching caregivers how to properly clock in and out for every shift.
Case Management & Monitoring
- Perform mandatory monthly follow-up calls with consumers to assess care quality, check caregiver performance, and address any service concerns.
- Travel to consumers' homes to conduct required annual in-person visits, completing quality assurance reviews, environmental assessments, and plan-of-care verifications.
- Maintain complete, audit-ready digital documentation for all monthly touchpoints and annual home visits.
Daily Operations & Scheduling Coordination
- Manage day-to-day scheduling adjustments, shift replacements, and timetable updates between consumers and PCAs.
- Monitor daily EVV clock-ins to identify missing shifts or missed clock-outs, troubleshooting timecard exceptions in real time.
- Act as the primary point of contact for daily phone calls, emails, and general consumer/caregiver inquiries.
Qualifications & Requirements
- Experience: 1–2+ years of experience in home care, Consumer Directed Services (CDS), or Medicaid administrative/coordination roles preferred.
- Travel Requirements: Valid driver’s license, reliable personal vehicle, and active auto insurance (required for annual local home visits).
- Tech Proficiency: Comfortable learning and navigating EVV software (e.g., Sandata, Ankota, or similar) and scheduling databases.
- Skills: Strong phone etiquette, excellent organizational skills, clear verbal/written communication, and a compassionate, patient approach.
