Job DescriptionWe are seeking a dedicated Home Health Registered Nurse to provide high-quality patient care in a home setting. The ideal candidate will have experience in skilled home care and be able to perform various assessments and interventions to promote recovery and rehabilitation of patients.
Responsibilities- Conduct patient assessments and develop individualized care plans.
- Administer medications and treatments as prescribed.
- Educate patients and their families on care protocols and wellness strategies.
- Monitor vital signs and report changes in patient conditions.
- Utilize adaptive devices and assistive technologies to improve patient mobility and safety.
- Document patient progress and maintain accurate medical records.
- Coordinate with interdisciplinary teams to optimize patient care.
- Minimum of 1 year of home health experience required.
- BLS certification is mandatory.
- State nursing license or compact license required.
- OASIS training preferred.
- Experience with electronic charting systems (Cerner preferred).
- Valid driver's license and reliable transportation for home visits.
- COVID-19 vaccination required or valid exemption.
- Flu vaccination required or valid declination with mandatory mask wearing during work hours.
