\n Graduate of an accredited Physical Therapy program
\n Bachelor’s degree if graduated before 1/1/2016
\n Doctorate of Physical Therapy (DPT) required if graduated after 1/1/2016
\n International candidates must have at least a Bachelor’s degree in Physical Therapy
\n Must hold or be eligible for an Indiana PT license within 90 days
\n CPR certification required within 90 days
\n APTA membership preferred
\n Experience preferred but not required beyond clinical training
