American Traveler is seeking an experienced RN Case Manager for a Care Coordination/Case Management role requiring a CA license and prior recent CA Dignity/CommonSpirit experience.

- Acute care setting in a Level 2 trauma hospital \n
- Care Coordination/Case Management unit covering all areas of the hospital, excluding outpatient and home care \n
- Handles 15-20 patients/consults per day across all payer types, including uninsured and homeless populations \n
- Uses Cerner EMR and Interqual criteria for utilization review \n
- 8-hour day shifts, scheduled between 8am-5pm depending on department \n
- Every other weekend required, with at least one full weekend per month \n
- Holiday coverage not required but welcomed if willing \n
- First to float among staff \n
- Business casual attire required \n
- Hospitalist/Intensivist and Respiratory Therapy available 24/7; pharmacy not available 24/7 \n

- Active CA RN license required \n
- BSN required \n
- BLS certification required \n
- Minimum 2 years of RN experience required \n
- Prior recent CA Dignity Health experience required for consideration \n
- Familiarity with Interqual criteria required \n
- Experience with both discharge planning and utilization review required \n
- Strong collaboration, teamwork, and communication skills with ability to reprioritize throughout the day \n
- Copy of driver's license required at time of consideration, reflecting legal name with no middle initial or middle name \n
- Two supervisor references required, one from within the last 12 months worked and one from within the last 3 years worked \n

- Responsibilities include completing initial assessments, utilization review (IQ), discharge planning, and charting in Cerner \n
- Both utilization management and discharge planning duties are required \n
- Training provided for Interqual (IQ) criteria \n
- 5 days of care coordination new employee orientation, with unit orientation timeframe varying case by case \n
- Candidates may not have been directly employed (full-time, part-time, or PRN) by any CommonSpirit-affiliated facility, including CHI or Dignity, within the past year \n
- Candidates employed PRN through outside staffing firms will be considered \n
