Job Description\n

The Physical Therapist provides general evaluations and treatments that significantly impact patients' functional abilities. This role includes overseeing and directing the work of Physical Therapist Assistants and other designated staff members.

Responsibilities\n

\n
• Conduct general physical therapy evaluations and treatments for patients of varying ages and diagnoses.
\n
• Assess patient needs, perform evaluations, identify therapy diagnoses, and develop treatment plans with measurable goals.
\n
• Implement, supervise, and modify therapy plans based on patient responses to optimize outcomes.
\n
• Review clinical records and case histories to inform treatment plans.
\n
• Engage in care conferences and regular meetings to discuss patient status and coordinate with healthcare professionals.
\n
• Collaborate with team members to arrange medical equipment and establish care plans prior to patient discharge.
\n
• Document patient encounters, including diagnoses, treatment plans, and recommendations in accordance with organizational policies.
\n
• Provide timely services in compliance with regulatory requirements.
\n
• Work with both in-patient and out-patient populations across various settings as assigned.
\n
• Supervise the work of support staff, students, or interns as needed.
\n
• Facilitate a partnership in healthcare between providers, patients, and their families.
\n
• Develop and conduct patient education and therapy programs tailored to individual or family needs.
\n
• Create clear home exercise programs for patients and ensure understanding through feedback and demonstration.
\n
• Advise patients and families on self-care methods to promote independence.
\n
• Assist senior staff with administrative duties and operations related to clinics and specialty areas.
\n
• Utilize hospital computer systems to access patient information and stay informed on directives.
\n
• Participate in information flow through emails and other digital platforms.
\n
• Provide summaries of results, statistics, and metrics as required.
\n
• Assist in fitting durable medical equipment/devices based on patient needs.
\n
• Communicate with supervisors and equipment providers to ensure proper measurement, fit, and delivery of equipment.
\n
• Maintain supply stocks and clinical equipment at appropriate levels.
\n
• Stay updated on specific billing procedures to assist in reimbursement for services.
\n
• Ensure cleanliness of the clinical environment.
\n
• Educate staff and patients regarding physical therapy-related issues and their conditions.
\n
• Participate in committee and management duties as needed.
\n
• Provide therapy consultations, presentations, and in-services as assigned.
\n
• Adhere to practice standards set by relevant professional organizations.
\n
• Follow confidentiality and quality assurance standards, participating in relevant functions and utilizing best practices.
\n
• Perform other duties as assigned.
\n

Qualifications\n

\n
• Entry-level qualifying master’s or doctorate degree in Physical Therapy from an accredited program or a graduate from an APTA-approved program with relevant experience.
\n
• Current license as a Physical Therapist.
\n
• Current Basic Life Support certification.
\n
• Valid driver’s license.
\n

Experience\n

\n
• Minimum of 1 year experience in adult outpatient rehabilitation.
\n

Shift Information\n

Monday to Friday, 8:00 AM to 5:00 PM. Holiday coverage is not required.
