As the Admissions Director, you will oversee the admissions process from initial referral through admission while developing relationships that support the facility’s census and occupancy goals.
You will serve as a key connection between the facility, referral partners, prospective residents, and families—helping ensure every admission is handled
with urgency, professionalism, compassion, and attention to detail.
Key ResponsibilitiesManage the admissions process from initial inquiry and referral through resident admission
Develop and maintain strong referral relationships with hospitals, physicians, case managers, social workers, discharge planners, assisted living communities, hospice providers, and other healthcare partners
Drive referral development and census growth through consistent outreach and relationship building
Review incoming referrals and conduct pre-admission screenings to determine appropriate placement
Coordinate verification of Medicare, Medicaid, Managed Care, commercial insurance, and private-pay coverage
Provide professional and welcoming facility tours to prospective residents and families
Educate residents and families about skilled nursing, rehabilitation services, insurance coverage, and the admissions process
Partner with Nursing, Therapy, Social Services, Business Office, and Administration to coordinate smooth and timely admissions
Maintain accurate and timely admissions documentation within the Electronic Medical Record (EMR)
Track and evaluate referrals, conversion rates, admissions activity, occupancy trends, and census goals
Identify opportunities to strengthen referral pipelines and improve admission conversions
Represent the facility through hospital outreach, networking, community partnerships, and professional events
Maintain compliance with applicable CMS, Medicare, Medicaid, state, federal, and facility requirements
Deliver an exceptional experience to every prospective resident, family member, and referral partner
Minimum of two (2) years of experience in healthcare admissions, healthcare marketing, business development, case management, referral development, or a related healthcare role
Experience within Skilled Nursing (SNF), Long-Term Care (LTC), Rehabilitation, Post-Acute Care, or a related healthcare setting
Knowledge of the skilled nursing admissions and referral process
Understanding of Medicare, Medicaid, Managed Care, and insurance verification
Strong communication, relationship-building, sales, and customer service skills
Ability to manage multiple referrals and priorities in a fast-paced healthcare environment
Strong organizational, follow-up, and problem-solving skills
Proficiency with Electronic Medical Records (EMR) and Microsoft Office
Bachelor’s Degree in Healthcare Administration, Business Administration, Marketing, Social Work, or a related field
Established relationships with hospital and community referral sources
Demonstrated success with census development, referral conversion, and occupancy growth
Previous experience as an Admissions Director or healthcare liaison
