About The Journey Ahead


The Journey Ahead Home Care Agency is a growing home care provider serving individuals and families throughout the Greater Cleveland area. We are looking for a personable, professional, and self-motivated Community Outreach & Referral Coordinator to help expand our community presence and develop meaningful relationships with organizations and professionals who serve individuals needing home care.

This is not a direct-care position. The primary focus is community outreach, relationship development, networking, and referral growth.

What You'll Do


The Community Outreach & Referral Coordinator will represent The Journey Ahead throughout the community and help establish the agency as a trusted home-care resource.

Responsibilities include:

  • Develop professional relationships with social workers, case managers, discharge planners, senior-service professionals, veteran organizations, healthcare facilities, assisted-living communities, rehabilitation facilities, senior housing communities, and other potential referral partners. 
  • Visit healthcare and community organizations throughout the Greater Cleveland area to introduce the agency and its services. 
  • Identify appropriate contacts within organizations and maintain ongoing relationships with referral sources. 
  • Research health fairs, senior expos, veteran events, community events, networking opportunities, and other events where the agency should have a presence. 
  • Coordinate and attend approved events, including setting up and staffing informational booths. 
  • Distribute agency brochures, referral information, business cards, and other approved marketing materials. 
  • Conduct professional follow-up by telephone, email, and in person. 
  • Identify potential referral opportunities and promptly communicate qualified leads to agency leadership. 
  • Maintain accurate records of outreach activities, contacts, follow-ups, referrals, and results. 
  • Provide weekly outreach reports to the Administrator. 
  • Represent The Journey Ahead professionally and accurately when discussing agency services and referral procedures.

  • Able to professionally represent the agency without direct supervision 
  • Comfortable using email, basic computer programs, and electronic tracking systems 
  • Able to maintain confidentiality and professional boundaries 
Preferred Experience


Previous experience in home care, healthcare marketing, community outreach, admissions, business development, senior services, social services, case management, healthcare sales, or referral development is strongly preferred.

Existing professional relationships within the Greater Cleveland healthcare, senior-services, Medicaid, veteran-services, or social-services community are a plus.

Requirements


Candidates must have reliable transportation, a valid driver's license, appropriate automobile insurance, and the ability to travel locally to referral sources and community events.

Some evening or weekend availability may occasionally be required for community events.
